you didn't mention the version that you fly...in my experience this started with one of the patches, maybe from patch 1.0.13954 on...I learned to be careful with the MP and the RPM, but it is a real pain sometimes in the finished game. So I would say to just pay very close attention to those two things to see if that is it...
if you are doing an air start, make sure that your throttle is set at a reasonable position before the beginning of the mission
