Personal feuds can spark wars between the factions. Lords can defect. Ladies can elope into marriage with a lord in another faction (this if anything will spark a war I highly recommend. EDIT: I have now played the game ~90 hours, and I have gathered enough troops/companions/experience to try and start my own kingdom, so I renounced my vassalage with the kingdom of Nords, and then laid siege and successfully conquered a castle from the currently weakened kingdom of Swadia. So I feel, that now, after about 90 hours play, the game is just beginning
