1: I believe it's Alt-Left click on the window, then right click and choose whatever option it is to remove the window. I could be wrong - all I know is I did it ages ago and they're not there anymore.
2: I think that's a bug, along with weapon loadouts not working for the QMB 3: also bugged as of the latest beta patch, iirc. There's a lot of iirc in this post. Others could confirm/deny.
