Well, I presume you post first, search later... anyway here are some extracts from various threads in here that may be of help:
Q: How do I set up my joystick?
A: On the Control Options screen, choose the "Axes" tab on the top of the screen. If you want to assign joystick buttons (or mouse/keyboard buttons/keys), choose the "Keys" tab.

The throttle goes to maximum even if my joystick throttle is only at 10-20%
A: Go to Control Options, Select Axes tab.
Select the throttle axis and look at the sliders for 0% Position and 100% Position.
Set for example 0.10 for 0% Position
and 0.90 for 100% Position
(This gives a bit of dead zone at both ends)

- Set the 5 sliders as follows, top to bottom:
*slider#1 = 0.02
*slider#2 = 0.00
*slider#3 = 1.00 <= all the way to the right
*slider#4 = 0.00
*slider#5 = 0.02
- Now the indicators on the right should move almost together, except for 2 small dead zones at both ends, to make sure that the game actually goes all the way down to 0% and up to 110% throttle
(to eliminate the dead zones set sliders #1 and #5 to 0.00)

OK, I got it figured out! I was on the right track but what I failed to realize was that my 1946 was on a fresh install and I hadn't set up my controller for it yet. After I set up my controller in 1946 I went into its config file and sure enough the proper values were now in there. I simply copied and pasted from the 1946 config to the CoD one. Here is what it looks like:

This, everyone, I have confirmed to work! You need the 1X values for it to work not the X values, the axes with the 1 infront are what windows uses to set the joystick sensitivity.
